One of the questions I hear most often after a treatment is some version of: when will I see my results? The honest answer is that the results patients ultimately see are not produced by me during the procedure. They are produced by the patient’s own biology over the weeks and months that follow. Understanding the science genuinely changes how patients experience the process.

What is collagen, and why does it matter?

Collagen is the primary structural protein of the skin, providing the scaffolding that gives skin its firmness and elasticity. Its progressive loss, which begins in our late twenties and accelerates through our forties and fifties, is the underlying cause of most visible skin ageing. Type I collagen forms thick, tightly organised fibres that provide tensile strength; Type III forms finer, more loosely woven fibres and predominates in newly healed tissue. The goal of effective skin treatment is to stimulate new Type I collagen, in the right location, at sufficient depth.

What does collagen stimulation actually mean?

The key cells are fibroblasts, the dermal cells responsible for synthesising collagen, elastin and hyaluronic acid. When the skin is injured, or when controlled thermal energy is delivered to the dermis, fibroblasts receive signals instructing them to increase their synthetic activity, triggering a cascade that unfolds in recognisable phases.

Inflammatory phaseGrowth factors signal fibroblasts to begin repair activity. Skin may appear red and swollen.

Early collagen synthesisNew collagen is synthesised. Type III predominates, forming the initial scaffold.

Remodelling phaseType III is progressively replaced by denser Type I. Most patients notice visible improvement here.

This is why I consistently tell patients that results at four weeks are not their final results, and results at three months are still not the complete picture. In many patients, the best results occur between six and nine months after treatment.

Why is the timeline so long?

Patients sometimes find it difficult to accept that the treatment they had today will not show its full effect for six months. In reality, a great deal is happening at a cellular level that is not visible in the mirror. Fibroblasts are actively synthesising new collagen fibres, cross-linking them for tensile strength, and progressively replacing early collagen with the denser Type I that gives skin its firmness. This process cannot be safely accelerated. It is governed by biology, not technology.

What patients often notice

Patients reviewed at three months are often pleased, and then reviewed again at six months are genuinely surprised by how much further things have progressed.

The treatments we offer are not surgical, and do not produce surgical results in the same timeframe. Results that develop over six months tend to look natural, because they are natural.
